OBJECTIVE:

To promote communication between dialysis facilities and transplant centers through implementation of clearly defined protocols that utilize a facility-level Transplant Liaison in the education of staff and patients about transplantation as a preferred modality for treatment of ESRD.

ACTIVITIES REQUIRED FOR DIAMOND STATUS:
  • Complete the in-service training module (PowerPoint Presentation) on Transplantation
  • 100% of staff complete pre- and post-tests
  • Use tracking mechanism to ensure that all new patients receive transplant education
  • Use tracking mechanism to ensure monthly lab samples are submitted for all patients on the transplant waitlist
  • Assign and submit the name of the healthcare worker (HCW) in the role of dialysis facility transplant liaison
OPTIONAL ACTIVITIES:
  • Distribute the Transplantation Glossary to facility staff and all eligible patients
  • Review your facility's policy and procedures for patient transplant education
  • View transplant educational video
  • Complete the word search exercise
