Patient Self-Managed Care

OBJECTIVE:

To establish a collaborative partnership between patients and healthcare professionals, to actively engage patients in their health and to integrate self-management skills to better cope with the challenges of living with and treating chronic illness.

ACTIVITIES REQUIRED FOR DIAMOND STATUS:
  • Complete an in-service training module (PowerPoint Presentation) on Patient Self-Managed Care.
  • Complete the Patient Self-Managed Care Perception & Confidence Scale Assessment with patients and discuss their role and reponsibilities.
  • Encourage patient participation in care plan meetings.
  • Complete Patient Self-Managed Care Perception & Confidence Scale Re-Assessment with patients.
OPTIONAL ACTIVITIES:
  • Distribute Live Your Best pamphlet to patients and their families in order to facilitate a patient-centered discussion about the importance of self-managed care.
  • Display 5As for Self-Management poster.
  • Complete Self-Management: Healthy Changes Plan worksheet with patients to assist them with short- and long-term healthcare goals.
  • Distribute and help complete Your Personal Health Record tool.
  • Complete the Patient Self-Managed Care Word Search.

  • 100% of staff in-serviced on Patient Self-Managed Care
  • 20% random sample of patients complete Patient Self-Managed Care Perception & Confidence Scale Assessment and the Re-Assessment
