Objective:
To implement a K-DOQI-recommended stenosis surveillance program in the dialysis unit that allows for a consistent and continuous evaluation process for AVF and AVG access.
Activities Required for Diamond Status/Measures:
100% of staff complete an in-service training module (PowerPoint Presentation) on stenosis surveillance.
100% of staff complete the Stenosis Surveillance Pre & Post Test Exercise.
Review your facility's policies and procedures for stenosis surveillance and identify opportunities for improvement.
Submit a de-identified stenosis tracking tool for trending stenosis surveillance.
