Objectives:

To establish a collaborative partnership between patients and healthcare professionals, to actively engage patients in their health, and to integrate self-management skills to better cope with the challenges of living with and treating chronic illness.

Activities Required for Diamond Status/Measures:

100% of staff complete an in-service training module (PowerPoint Presentation) on patient self-managed care.
20% random sample of patients complete the Patient Self-Managed Care Perception & Confidence Scale Assessment and Patient Self-Managed Care Perception & Confidence Scale Re-Assessment.
Encourage patient participation in care plan meetings

Optional Activities:

Distribute Live Your Best pamphlet to patients and their families in order to facilitate a patient-centered discussion about the importance of self-managed care.
Display 5 As for Self-Management poster.
Complete Self-Management: Health Changes Plan worksheet with patients to assist them with short- and long-term healthcare goals.
Distribute and help complete Your Personal Health Record tool.
Complete the Patient Self-Managed Care Word Search.

Completed Project Reporting Form
In-service attendance log
De-identified copies of the completed Patient Self-Managed Care Perception & Confidence Scale Assessments (English or Spanish) and the Re-Assessments (English or Spanish)
