Patient Safety Principles (Mandatory Module)

Objective: 


To learn the basic concepts of patient safety in a just culture environment and what proactive steps can be used to prevent errors.


Activities Required for Diamond Status/Measures:

  • 100% of staff complete the 8-question safety culture questionnaire which should be shared with staff and leadership before in-service training
  • 90% of staff participate in in-service training with PowerPoint presentations  What is Patient Safety? and Patient Safety in the Dialysis Unit
  • Submit completed Project Reporting Form


Optional Activities:

  • Retest staff six months after the 5-Diamond Program has been completed
  • Display Patient Safety posters
  • Distribute Speak Up brochure to patients
  • Develop a patient safety policy
  • Utilize organizational assessment tool for a culture of respect, communication, and disclosure.
