Objective:


To improve communication between staff and patients, thereby building a trusting and cooperative partnership in care.
 

Activities Required for Diamond Status/Measures:

  • 100% of staff complete an in-service training module using at least one Communication presentation AND at least one Professionalism presentation (listed in the Tools/Resources section)
  • Encourage and invite 100% of patients to participate in care plan meetings.
  • Invite 100% of patients to complete satisfaction surveys.
  • Utilize results from the Patient Satisfaction Assessments/Surveys (listed below) in QAPI.


Optional Activities:

  • Perform sensitivity training with all staff using the Guided Imagery Exercise

After you have completed the Communication module and have documented the required measures, please complete and submit to your Network Office:
  • Completed Project Reporting Form
  • In-service attendance log
  • Sample copy of patient care plan invitation
  • Sample copy of invitation to patients to complete satisfaction survey
  • Data indicating rate of satisfaction survey return and results
  • Minutes from QAPI sessions utilizing patient satisfaction survey results
